Paper form or online registry
There are two ways your result reaches the DMV. A provider enrolled in the DMV’s Online Vision Registry sends it electronically. Otherwise, a qualified provider fills in the paper Vision Test Report (form MV-619), which you bring to the DMV or mail with your renewal.
The registry also matters if you no longer need glasses to drive, for example after eye surgery: when an enrolled provider records a passing result without correction, the DMV removes your corrective-lens restriction when you renew online.
A vision test is not an eye exam
The DMV test checks one thing: whether you can read the chart well enough to drive. It doesn’t check eye health, so it won’t catch problems like glaucoma or cataracts. Keep seeing an optometrist or ophthalmologist on the schedule they recommend.
