Why it happens

• The drug is expensive or has cheaper alternatives on the formulary
• The plan requires you to try another drug first (step therapy)
• The dose or quantity is higher than the plan’s limit
• Your plan changed its formulary on January 1

What happens next

1. The pharmacy gets a rejection when it bills your plan.

4. The plan decides. Standard decisions often take a few days; urgent requests can be faster.
5. If approved, we fill it and deliver it. If denied, your doctor can appeal or choose a covered alternative.
